What to Bring: Medical Release & Player Registration Proof – If you played for Texas Rush during the 2016/17 season, the Admin staff will verify your player registration. A medical release form is required for every player at check-in regardless of club affiliation. The Texas Rush office does not maintain a copy of player medical release forms. The forms are carried to each game by the competitive coach or team manager. Texas Rush Recreational players will need to complete the STYSA medical release form, see link below. If you are unsure which form your players need to complete, please contact your club office.
